How do I register a complaint against a D.O. and what happens after that? The first step is to mail your written complaint to the Board. You will find a Report Form on this website under Forms (for the Public) which has the mailing address. Step 2: All Reports (complaints) are sent to a Case Review Committee comprised of the Executive Director, Board Attorney, Medical Advisors and Investigators.
